The goal of the Choosing Wisely Champions program is to help expand the Choosing Wisely campaign through positive stories of
individual clinicians or teams of clinicians. Champions inspire others so that everyone can apply best practices in their own
environments. Thank you for your interest in nominating yourself or a colleague as the AGS's Choosing Wisely Champion.
This application will allow you to select, and expound upon, characteristics that define a Choosing Wisely Champion.
Nominees will be notified in March of the outcomes of the selection process. If you have any questions, please contact Mary Jordan Samuel at mjsamuel@americangeriatrics.org.

The AGS Choosing Wisely champion should demonstrate at least two (2) of the following characteristics:
• Advanced Choosing Wisely in their practice.
• Played a major role in development of society recommendations.
• Helped educate society members about Choosing Wisely and/or recommendations.
• Conducted measurement of recommendations.
• Validated research of importance of recommendations.
• Demonstrated leadership of a localized Choosing Wisely effort.
• Served as a spokesperson for the society or campaign.
Champion resources and rewards include:
• A certificate presented at the 2019 AGS Annual Scientific Meeting awards ceremony.
• A $50 gift certificate to GeriatricsCareOnline.org.
• Potential features on the Choosing Wisely website and in select Choosing Wisely resources.
• Features in AGS communications (press release, newsletter, listserv, etc.).
• Template materials (email announcement, sample press release, etc.) for sharing news.
