DESCRIPTION

The AGS Age-Friendly Resource Library provides resources based on the 4Ms framework of an Age-Friendly Health System from the Institute for Healthcare Improvement (IHI), AGS, and the AGS Health in Aging Foundation (HiAF). These resources are designed to support your work on the path to becoming age-friendly and focus on different aspects of the 4Ms Framework - What Matters, Mentation, Medication, and Mobility. Each of the 4Ms is divided into its own section and is separated between professional & public resources. The toolkit also includes sections on the Business Case for Becoming an Age-Friendly Health System and Cross-Cutting resources.
