TARGET AUDIENCE:  Physician, Medical Director, Nursing, Pharmacy, PA’s, Fellows, Residents, & Medical Students.

 

OBJECTIVES:  Learn which classes of medications present with highest adverse drug events (ADE) in hospital setting and how to apply evidence-based algorithms & protocols for deprescribing;  Understand complexity/problems created by transitions of care with process of transfer of orders and risks of medication errors; Apply strategies for communicating medication changes with nursing team, pharmacist, patients, families, and other health professionals; Identify risks/benefits to select potentially inappropriate medications (PIM) and ADE; Learn evidence-based tools for outpatient setting;  Understand high-risk classes of medications and how to select a class of medication to reduce tapering and stopping a specific high-risk class;  Understand basic principles of deprescribing, including drug cascades, drug syndromes, ADE, PIM, and guidelines including STOPP/BEERS;  Understand role of pharmacist in deprescribing process, including QAPI and scenarios to help achieve best outcomes for patients with multiple prescribing health professionals.
