The 17th annual Rocky Mountain Geriatrics Conference will focus on "Creating an Age-Friendly Health System: The 4 Cornerstones”  It is being held outside of Salt Lake City at the Snowbird Ski and Summer Resort on August 26-27.

The conference features three Keynote Speakers: 

  • Rebecca Sudore, MD: Professor of Medicine, University of California, San Francisco
  • Cynthia J. Brown, MD, MSPH, AGSF: Professor of Medicine & Director, Division of Gerontology, Geriatrics and Palliative Care, Comprehensive Center for Healthy Aging, University of Alabama at Birmingham
  • Sunny Linnebur, Pharm.D, FCCP, FASCP, BCPS, BCGP: Professor, University of Colorado Skaggs School of Pharmacy and Pharmaceutical Sciences, University of Colorado Anschutz Medical Campus

We invite professionals across health care disciplines and professionals working in geriatric fields to become familiar with the 4M cornerstones of an Age-Friendly Health System: What Matters, Medication, Mentation, and Mobility.  These essential, evidence-based, interventions address the complex needs of older adults with the goal of delivering high quality and age-friendly care across settings of care -  including hospitals, primary care, home health, and other settings. Each component of the 4Ms will be explored in order to improve care.  Participants will leave the conference with practical tools to provide, and optimize, safe, effective, and patient-centered care.  They will discover how to operationalize and integrate the 4Ms into health systems to identify the goals of patients, promote physical function, address cognitive health, and prevent polypharmacy, and to provide connections to community resources.
