Leaders in PALTC will share their insight into Special Needs Plans (SNP), a program of financing and service delivery under Medicare Advantage that is growing rapidly in our setting.
Objectives
* Medicare Advantage background
* Overview of the SNP model of care and use the PALTC setting
* Success stories of keeping residents out of the hospital during the COVID-19 pandemic
Listen in as Cheryl Phillips, President and CEO of the SNP Alliance, will describe the various models of SNPs that exist and their applicability in PALTC. She is joined by Ronald J. Shumacher, Chief Medical Officer for Optum Senior Community Care and National Medical Director for Optum Home & Community Care and Senior Community Care and Jamie Babcock, Vice President of Clinical Field Operations for Optum Senior Community Care who will share their insight about the Institutional Special Needs Plan (I-SNP) that Optum has provided in PALTC for many years.
