The AGING Initiative in collaboration with Oregon Health and Sciences University is hosting a webinar on December 3, 2024 at 10am PT/1:00pm ET. Ana Quiñones, PhD and David Dorr, MD will be presenting on "Preliminary Findings From Person-Centered Care Planning for People with Multiple Chronic Conditions (PCCP4P)." This webinar will discuss findings from the PCCP4P project, including a high-level overview of models and approaches to providing person-centered care planning (PCCP) for people with multiple chronic conditions (MCCs). We will highlight learnings, gaps, persistent barriers, and promising solutions to the adoption and promotion of high quality PCCP.
