Integrating Health Care and Social Services for People with Serious Illness: A Workshop

n July 19, 2018, the Roundtable on Quality Care for People with Serious illness will host a public workshop on the integration of health care and social services for people with serious illness.The workshop will feature invited presentations and panel discussions on topics that may include:

  • the range of health care and social services (e.g., medical/dental, psychosocial/spiritual, nutrition, transportation, housing, home care, caregiver assessment and training) necessary to improve care for those with serious illness
  • the strengths, limitations, and impact of existing models of integrated services delivery (e.g., Programs of All-Inclusive Care for the Elderly [PACE], Special Needs Plans [SNPs]), the Bridge Model, and novel, innovative approaches to community-based integrated care programs
  • the gaps in and barriers to the delivery of integrated services
  • potential use of blended/braided financing approaches
  • the role of family caregivers in providing  social supports and services, and caregiver needs (e.g., emotional, financial, training, respite care)
  • gaps in research and evidence to support potential policy changes to advance the integration of health care and social services deliver
