The Home Centered Care Institute invites you to join us on December 9th at 5pm CT for an important discussion about the need for – and tremendous value of – home care medicine or the modern-day “house call". Please join the conversation led by:
- Dr. Tom Cornwell is a nationally recognized house call leader who has worked with Congress,
the Center for Medicare and Medicaid Services (CMS), and its Innovation Center (CMMI) to
advance home-based primary care. He will discuss the local and national progress towards
making this model of care more accessible to those who need it.
- Dr. Elizabeth Davis leads the newly expanding house call program, Rush@Home, at Rush
University Medical Center, An HCCI Practice Excellence Partner. The program is having a
powerful impact on vulnerable patients and their caregivers who have historically lacked equal access to healthcare, this has been especially true during the pandemic.
- Julie Sacks, who leads HCCI, will tell you about the organization’s mission, vision, and
key initiatives and how you can join us in our efforts to make house calls part of the
healthcare mainstream.
