Patient Priorities Care Fall Tele-Mentoring Series

This virtual course outlines the Patient Priorities Care (PPC) decision-making framework for persons with multiple chronic conditions and supports learners with expert mentorship to apply the approach in their practice. PPC is an approach to decision-making that aligns health care with a person’s own health priorities – the health outcome goals they most desire, given the healthcare tasks they are willing and able to do to achieve them.

 

Dates and Times: Every other Tuesday (Oct 8-Dec 17) from 2-3 p.m. EST.

Why it matters: PPC is an evidence-based, feasible clinical practice associated with improved outcomes including:

  •  Reduced treatment burden
  •  Reduced unwanted care
  • Increased desired care

The core content of the course will be delivered over 6 sessions, with an optional seventh session geared towards those (with or without formal roles in education) who wish to teach this framework to other health professionals or trainees.

Who should take this course? Those seeking Age-Friendly Health System designation, and all health professional leaders and educators who want to transform clinical care to focus on What Matters or carry out curricula to educate others to provide care that aligns with What Matters. This training addresses all 4Ms (What Matters, Medication, Mentation, Mobility) through the context of What Matters.

Certification: Participants who miss no more than 1 live, virtual session will receive an “Expert in Patient Priorities Care” certificate. Sessions will be recorded, but live attendance is required to receive certification.

CEUs: FREE! 6 CE credits for live attendance at the full series for the following disciplines: JA IPCE, ACCME, ACCME-NP, ANCC, NBCC,  ACPE, APA, NYSED P, ASWB, NYSED SW, AAPA, APTA, AOTA
